The following application is quite extensive. This application has been designed to allow you to give us as much information as possible about your organization. This will allow us to select the medical supplies and equipment that will be the most beneficial to your facility. We realize this is a comprehensive application, but as the value of each donated shipment is approximately US$250,000.00 we feel it is necessary. It is not mandatory that you answer every question but the more information you provide, the greater the likelihood of your request being approved.

If you are applying for the first time, you will need to establish a Rotary METS Web account. During this process you will establish a username and password. Use your login credentials to save applications as you progress through the information. Return to unfinished applications at anytime. Your application will not be submitted for review until the last section and all required fields have been completed.
